The following will be the fee's for Fall 2007 Season

7-week regular season and Fall tournament:

$100 ICSL per Community fee
$7.00 Each Player (IYSA insurance fee)*
$231 per Jr. High and U12 Team (referee fee)
$214 per U10 Team (referee fee)
$200 per team for Fall Tournament games/awards

*IYSA insurance fee is good for Fall/Spring season per registered player

The following (6) forms are due by the league paperwork deadline:

            (1)    IYSA Emergency Medical Release & Liability Waiver Form (For EACH player)
            (2)    IYSA Organization Statement Of Understanding And Agreement Form
            (3)    IYSA Disclosure Statement Form (For EACH COACH that did not submit one in 2006 OR 2007)
            (4)    ICSL Team Roster Form (For EACH Team)
            (5)    IYSA Roster Membership Spreadsheet
            (6)    Coaches Code of Conduct Agreement (For EACH coach)
