PAPERWORK SUBMISSION DIRECTIONS

ICSL – Spring 2018

In order to assist the secretary of the ICSL in reviewing the large amounts of paperwork received by each community for the upcoming season, please follow the instructions below for submitting the paperwork for your community.  All forms are attached and can be downloaded from the “Coach’s Corner” link on the ICSL web page.

The final packet of information being submitted should be stacked from top to bottom as listed below

·         Check –The check should be paper-clipped to the Cover Sheet. Make checks payable to the ICSL or Inter Community Soccer League.  Please be sure you have checked for accuracy.  The check will need to include all items covered on the Cover Sheet including the single $100 fee per community regardless of the number of teams being registered.  One check should be issued per community.

·         Cover Sheet – As indicated by its title, this sheet should be at the top of your packet of registration materials.  Please be sure that the sheet is completed accurately and includes calculations for the $100 community fee, new player fees and team fees. One cover sheet should be issued per community. Please Note - After lengthy discussions with IYSA President Mary Jane, it has come to the attention of the ICSL board that "tournament only" player cards as well as "CIYSL" player cards do NOT provide insurance for players to participate in the ICSL.  In an ongoing effort to assess and improve safety/risk management for the league, we are changing, effective immediately, the requirement regarding player insurance.  The ICSL board will be requiring all players participating in the Inter-Community Soccer League to register for IYSA coverage through the ICSL, meaning that each community will need to pay the $9 per player fee. No exceptions. The cover sheet has been updated for Fall 2017 to reflect this change.

·         Illinois Youth Soccer Association Organization Statement of Understanding - This form should be completed and included after the Cover Sheet for your community.

·         Team Information – After the IYSA Organization Statement of Understanding, the packet of registration information for each team competing in the fall will need to be included.  Registration information for each team should be itemized in the following manner.

o   Roster – After entering the roster for each team on the GotSoccer website, follow the instructions to generate a roster for download into a PDF format.  Print a copy of that roster to include in this packet.  Please be sure the roster includes the names of the coaches who will be coaching with the team.

o   Emergency Medical Release and Liability Waiver Forms – One form needs to be completed and submitted for each child listed on the roster.  AND COPIES OF ALL PLAYERS BIRTH CERTIFICATES.

o Coach’s Code of Conduct Forms – It is only necessary to submit the final signature page for each coach listed on the team’s roster.  Remember that in addition to completing this form, a coach must ANNUALLY complete the criminal background check online.

PLEASE READ AND FOLLOW THE INSTRUCTIONS BELOW TO COMPLETE THE  BACKGROUND CHECK EVERY TWO YEARS on the GotSoccer website.

o ALL Coaches MUST submit copies of their Concussion course completion certificate.  This concussion course only needs to be done once(Per IYSA, at this point).  If you completed and do not have a copy of the certificate, you will need to take the course again. This course takes about 15mn to complete.

Submit the packet of the information below starting with any U14(Jr High) team you may have, followed by any U12, U10, U11 and U9 teams.  High School teams are Spring only. For each team, the team information should be paper-clipped together by team.

Remember – Be sure to make a copy of all paperwork submitted.  Coaches will need to keep a copy of the rosters and emergency medical release and liability forms.  Coaches will be required to show player cards and a roster before each game so referees can check them.  If coaches do not have cards and rosters, the team will not be allowed to play that day.

Submission – All paperwork should be received by March  12, 2018.  Submit all paperwork to:
